{"id":6470,"date":"2020-02-11T15:01:23","date_gmt":"2020-02-11T14:01:23","guid":{"rendered":"https:\/\/federicogiardina.com\/2020\/02\/11\/painful-knee-replacement-why-it-happens-and-how-to-remedy-it\/"},"modified":"2025-10-06T17:20:57","modified_gmt":"2025-10-06T15:20:57","slug":"painful-knee-replacement-why-it-happens-and-how-to-remedy-it","status":"publish","type":"post","link":"https:\/\/federicogiardina.com\/en\/2020\/02\/11\/painful-knee-replacement-why-it-happens-and-how-to-remedy-it\/","title":{"rendered":"Painful knee replacement: why it happens and how to remedy it"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">The knee is a joint governed by many ligaments (in fact, only flexion-extension is allowed) and endowed with rich proprioceptive innervation; this causes postoperative mobilization of the <a href=\"https:\/\/federicogiardina.com\/en\/?page_id=6414\">prosthesis <\/a>to be characterized more frequently by<strong> pain<\/strong>, a condition that occurs more frequently than for the hip, as in <a href=\"https:\/\/federicogiardina.com\/en\/?page_id=6443\">hip<\/a> <a href=\"https:\/\/federicogiardina.com\/chirurgia-dell-anca\/\">replacement<\/a> or hip arthroscopy.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>This pain<\/strong>, present in the first few days after surgery, <strong>should not be frightening<\/strong>; in fact, it <strong>gradually disappears<\/strong>, giving way to proper functional recovery of the knee.<\/p>\n\n<p class=\"wp-block-paragraph\">If, on the other hand, postoperative <a href=\"https:\/\/federicogiardina.com\/en\/?page_id=6414\">knee replacement<\/a> pain persists over time, changes its characteristics, and cannot be controlled through pain medication, it is possible that the replacement may have a problem.<\/p>\n\n<p class=\"wp-block-paragraph\">It is advisable to <strong>see your<\/strong> orthopedic <strong>surgeon immediately<\/strong> if:<\/p>\n\n<ul class=\"wp-block-list\"><li><strong>pain persists beyond six weeks;<\/strong><\/li><li><strong>intense burning is felt;<\/strong><\/li><li><strong>sudden twinges and\/or sags occur.<\/strong><\/li><\/ul>\n\n<h2 class=\"wp-block-heading\">Painful knee replacement or infected knee replacement?<\/h2>\n\n<p class=\"wp-block-paragraph\">The first thing to rule out when the function of a knee replacement gets worse instead of better is the <strong>presence of an infection<\/strong>.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Periprosthetic infection<\/strong> is certainly the most fearsome of the complications, as well as the one that may require the longest course for recovery.<\/p>\n\n<p class=\"wp-block-paragraph\">By performing <strong>blood physical examinations<\/strong> and possibly <strong>ultrasound sampling from the joint<\/strong>, this rare but extremely dangerous complication can be ruled out.<\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/01\/Injection-acid-hyaluronic.jpg?resize=546%2C307&#038;ssl=1\" alt=\"\" class=\"wp-image-772\" width=\"546\" height=\"307\" srcset=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/01\/Injection-acid-hyaluronic.jpg?w=546&amp;ssl=1 546w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/01\/Injection-acid-hyaluronic.jpg?resize=300%2C169&amp;ssl=1 300w\" sizes=\"auto, (max-width: 546px) 100vw, 546px\" \/><\/figure><\/div>\n\n<p class=\"wp-block-paragraph\">It is very important to <strong>recognize a prosthetic infection early<\/strong> because, in this way, it can be treated without the prosthesis having to be removed. If, on the other hand, the elapsed time was already too long (more than 30 days), a surgical cleaning coupled with antibiotics will fail to eradicate it, and it will be necessary to remove the prosthesis and then implant it again after healing.<\/p>\n\n<h2 class=\"wp-block-heading\">Identify the cause of painful knee replacement<\/h2>\n\n<p class=\"wp-block-paragraph\">Excluding the possibility of an ongoing infection, it is possible that, in the first few weeks, the pain felt during walking may be due to <strong>not fully functional recovery<\/strong>.<\/p>\n\n<p class=\"wp-block-paragraph\">It will, therefore, be necessary to insist on <strong>rehabilitation<\/strong>, looking, in parallel, for the mechanical causes of pain.  <\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/02\/REhab-Knee.jpg?resize=478%2C319&#038;ssl=1\" alt=\"\" class=\"wp-image-900\" width=\"478\" height=\"319\" srcset=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/02\/REhab-Knee.jpg?w=637&amp;ssl=1 637w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/02\/REhab-Knee.jpg?resize=300%2C200&amp;ssl=1 300w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/02\/REhab-Knee.jpg?resize=600%2C400&amp;ssl=1 600w\" sizes=\"auto, (max-width: 478px) 100vw, 478px\" \/><\/figure><\/div>\n\n<p class=\"wp-block-paragraph\">The first thing will be to perform a thorough <strong>clinical and medical history examination<\/strong>: knowing where and when the pain appears is of primary importance in reaching a diagnosis.  <\/p>\n\n<p class=\"wp-block-paragraph\">Subsequently, the progression of radiographic images and, possibly, a CT scan (with metal artifact exclusion protocol), are of great help in highlighting the cause of prosthesis malfunction.<\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" \/><\/figure><\/div>\n\n<h2 class=\"wp-block-heading\">Knee replacement: therapies in case of pain<\/h2>\n\n<p class=\"wp-block-paragraph\">Before considering surgical revision, it is essential to rely on renewed <strong>physical therapy<\/strong> as well as <strong>antalgic <\/strong>(pain reduction) <strong>therapy <\/strong>: placement of a <strong>perinervous catheter<\/strong> or use of <strong>antalgic radiofrequency<\/strong> allows pain-free functional recovery.<\/p>\n\n<figure class=\"wp-block-image size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" physiatrist, physical therapist, and sometimes the pain therapist.<\/p>\n\n<h2 class=\"wp-block-heading\">The surgical revision<\/h2>\n\n<p class=\"wp-block-paragraph\">If a definite cause has been identified that can explain the persistent knee replacement pain and, despite repeated physical and antalgic therapies, good function of the joint has not been achieved, then it will be necessary to
