therefore, today, research channels much of its efforts into trying to establish a close correlation between knee and hip healing and <strong>Regenerative Medicine.<\/strong> <\/p>\n\n<p class=\"wp-block-paragraph\">When Regenerative Medicine is applied in Orthopedics, it takes the name <strong>Orthobiology<\/strong>, a discipline that aims to <strong>avert joint degeneration<\/strong> by using the patient&#8217;s own tissues and cells.<\/p>\n\n<p class=\"wp-block-paragraph\">But let&#8217;s go in order.<\/p>\n\n<h2 class=\"wp-block-heading\">REGENERATIVE MEDICINE: let&#8217;s get some clarity.<\/h2>\n\n<p class=\"wp-block-paragraph\">When we talk about <strong>regenerative medicine<\/strong>, we mean that branch of medicine that aims to repair a damaged tissue through <strong>cellular regeneration<\/strong> and not through metal replacement of the joint.<\/p>\n\n<h4 class=\"wp-block-heading\">The &#8220;birth&#8221; of mesenchymal cells<\/h4>\n\n<p class=\"wp-block-paragraph\">It is based on the use of a particular type of cells, called Mesenchymal Stem <strong>Cells <\/strong>(literally mesenchymal stem cells). <strong>This term was coined in 1991 by the American scientist Arnold Caplan<\/strong>: he envisioned a class of human bone cells that could be isolated and multiplied in culture while retaining <strong>the ability to differentiate into different tissues such as bone, cartilage, <\/strong>adipose tissue to others.<\/p>\n\n<figure class=\"wp-block-image size-large\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" width=\"700\" height=\"221\" src=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=700%2C221&#038;ssl=1\" class=\"wp-image-1658\" title=\"Article by Arnold Caplan\" srcset=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=1024%2C323&amp;ssl=1 1024w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=300%2C95&amp;ssl=1 300w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=768%2C242&amp;ssl=1 768w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=700%2C221&amp;ssl=1 700w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?resize=1269%2C400&amp;ssl=1 1269w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.001.png?w=1383&amp;ssl=1 1383w\" sizes=\"auto, (max-width: 700px) 100vw, 700px\" \/><\/figure>\n\n<p class=\"wp-block-paragraph\">This ability of theirs is called <strong>multipotent<\/strong>, and it <strong>mistakenly<\/strong> became the evidence for calling them precisely <strong>stem cells<\/strong>.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Today after hundreds of clinical trials, the regenerative ability of these so-called stem cells (Mesenchymal Stem Cells) has been greatly scaled down.<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">In fact, it has been shown from the numerous clinical studies that these cells <strong>are unfortunately not stem<\/strong> cells, thus endowed with differentiative and regenerative capacity.<br\/>The positive clinical results are due to the fact that these cells, usually obtained from bone marrow (from the iliac crest of the pelvis) or from the patient&#8217;s subcutaneous fat, are <strong>pericytes (cells derived from small vessels)<\/strong> that, when injected into a new environment (hip or knee), <strong>are activated by releasing molecules that reduce inflammation: <\/strong>this causes the joint environment to return to its physiological function, <strong>decreasing pain<\/strong>.<\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.003.png?resize=583%2C214&#038;ssl=1\" class=\"wp-image-1660\" width=\"583\" height=\"214\" title=\"Mesenchymal Stem Cells and the Wound Healing Process. Cells 2013, 2, 621-634  <\/figcaption><\/figure><\/div>\n\n<p class=\"wp-block-paragraph\">Therefore, they are mainly used for injuries involving cartilage, such as, indeed, coxarthrosis and gonarthrosis.  <\/p>\n\n<p class=\"wp-block-paragraph\">Recently, Prof. Caplan expressed his willingness to change the term he coined (Mesenchymal Stem Cells), replacing it with <strong>Medicinal Modulating Cells (Medicinal Signaling Cells).<\/strong><\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=582%2C242&#038;ssl=1\" class=\"wp-image-1659\" width=\"582\" height=\"242\" title=\"Article by Arnold Caplan\" srcset=\"https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=1024%2C426&amp;ssl=1 1024w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=300%2C125&amp;ssl=1 300w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=768%2C320&amp;ssl=1 768w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=700%2C291&amp;ssl=1 700w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?resize=961%2C400&amp;ssl=1 961w, https:\/\/i0.wp.com\/federicogiardina.com\/wp-content\/uploads\/2020\/06\/Ortobiologia.002.png?w=1268&amp;ssl=1 1268w\" sizes=\"auto, (max-width: 582px) 100vw, 582px\" \/><\/figure><\/div>\n\n<p class=\"wp-block-paragraph\">In fact, as mentioned earlier, it has been shown that these cells, released into the joints, have the sole ability to &#8220;modulate&#8221; the damaged environment by &#8220;talking&#8221; to their surroundings.<br\/><strong>This communication allows the slowing of cell apoptosis (the physiological cell death) of chondrocytes <\/strong>and promotes the synthesis of collagen, the building block of &#8220;cartilage.&#8221;<\/p>\n\n<p class=\"wp-block-paragraph\">This occurs especially in moderate arthritic conditions.<\/p>\n\n<p class=\"wp-block-paragraph\">It is possible to date to say that these are safe procedures; but then how effective are they?  <\/p>\n\n<h2 class=\"wp-block-heading\">THE APPLICATION IN ORTHOPEDICS: ORTHOBIOLOGY<\/h2>\n\n<p class=\"wp-block-paragraph\">We mentioned that<strong>Orthobiology<\/strong> is that discipline that combines Regenerative Medicine and Orthopedics; its goal is to try to <strong>recover what is damaged at the joint level<\/strong> rather than proceeding immediately with surgery.  <\/p>\n\n<p class=\"wp-block-paragraph\">Obviously, this type of choice is made when faced with the early stages of joint degeneration.  <\/p>\n\n<h3 class=\"wp-block-heading\">ORTHOBIOLOGY: HOW IT WORKS<\/h3>\n\n<p class=\"wp-block-paragraph\">When the first symptoms of hip or knee osteoarthritis begin to appear, <strong>cell precursors are harvested<\/strong> from adipose tissue, bone marrow, and subcutaneous tissue.  <\/p>\n\n<p class=\"wp-block-paragraph\">These particular cells are then introduced by infiltration inside the damaged joints; thus, the concentration of precursor cells in the hip or knee increases.  <\/p>\n\n<h3 class=\"wp-block-heading\">the benefits of orthobiology<\/h3>\n\n<p class=\"wp-block-paragraph\">By introducing precursor cells into the joints, cells already present in the cartilage are &#8220;reactivated&#8221;; this promotes their regeneration, allowing them to be <strong>blistered and reduce<\/strong> hip or knee joint <strong>pain<\/strong>. However, we know that <strong>if the damage is not advanced, the mesenchymal cells can form a cartilage-like tissue that integrates properly with the surrounding tissue<\/strong> through type 2 collagen.<\/p>\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" said, however, should by no means discourage: the results of infiltrative therapy in the orthopedic field for the early stages of osteoarthritis are obvious.<\/p>\n\n<p class=\"wp-block-paragraph\">We will need to identify the Patient&#8217;s characteristics to use the most sensible treatment approach, choosing the use of infiltration, or surgical options such as <a href=\"https:\/\/federicogiardina.com\/en\/fai-and-artroscopy-of-the-ankle\/\">hip arthroscopy <\/a>or <a href=\"https:\/\/federicogiardina.com\/chirurgia-dell-anca\/\">hip replacement<\/a>.<a href=\"https:\/\/federicogiardina.com\/en\/fai-and-artroscopy-of-the-ankle\/\"> <\/a><\/p>\n\n<p class=\"wp-block-paragraph\"><a rel=\"noreferrer noopener\" aria-label=\"terapie infiltrative (apre in una nuova scheda)\" href=\"https:\/\/federicogiardina.com\/en\/infiltrative-therapies\/\" target=\"_blank\">Infiltrative therapies<\/a> are completely safe and have the ability to reduce pain, improve joint function, and consequently increase quality of life, and can be combined with conservative surgical options such as<a
